Single- Supplier Framework: Co-occurring Conditions Support Programme The service will deliver coordinated, integrated support for people experiencing multiple disadvantage, with a defined focus on those facing co-occurring homelessness, substance misuse and mental ill health. It is expected to maintain or enhance existing staffing capacity of current contracts, while breaking down workforce barriers to create a more joined-up system across current programmes and other existing services. Providers will integrate within the Enhanced Live Well offer, work collaboratively with other inclusion health services across Greater Manchester, and take on a lead role in building partnerships—both within this contract and across the wider system—to improve strategic alignment and workforce support.
